Chamilo LCMS Connect » Courses

I've created a course and in that course I've published an announcement.
When you are in a course you can select announcements and then you select one.
Then you can see different icons on the right of each announcement I've selected the one with the magnifying glass.
Now you see in the action bar a link 'create feedback' you have to click on that.
Then you get the possibility to give a title, description and an attachment, I've filled in both and added an attachment.
When you clicked on create you will go to a page with two chamilo headers/menu's and you'll get this error
'headers already sent in /var/www/chamilo/.... on line 136.
