or: default course setting: “subscribe not allowed”

the default setting in a course is now: subscription allowed = true; unsubscription allowed = false.
Since we don't allow our students to subscribe to a course (subscription is done by the admin with a csv), we would like to have a setting at the admin level to set "subscription allowed" to false.
Another solution for us could be to hide "subscribe/unsubscribe" in the "user course management" (issue 699).

Note that we will remove these settings from the admin panel because they will be changed to course types, more info will be available later about course types.
