From a pedagogical point of view, i find this layout to be better. It offers better ergonomy and less scrolling.

Yes. We usually use the test settings to add texts, sounds or images so that they appear on every page/question of a test. Even like that, it isnt an ideal solution since it requires a lot of scrolling up and down for students.

The Dokeos direction now is to do a lot of small fixed changes that are not really portable, nor flexible, nor extensible (to reduce costs), so I wouldn't really inspire myself too much from the Dokeos code.
I agree with Julio that there should be a "layout option" or something like that that changes the way a specific question is shown, so that it is "classic" style or "boxes" style.
