Description

Assessment with 4 questions, configured to show 3 random questions. Taking the assessment is ok, the system shows 3 questions.
After submitting my answer two errors:

- in the feedback view (see screenshot) there is suddenly the 4th question that was not shown in the assessment (and of course I receive no points)
- in the feedback view my third question is missing.

Seems like the assessment module does not save the questions that where shown to the end user.
